6lt%3B%5C%2Fdiv%26gt%3B%5Cn%26lt%3B%5C%2Fdiv%26gt%3B%22%2C%22data%22%3A%7B%22itemType%22%3A%22journalArticle%22%2C%22title%22%3A%22Augmented%20Stress%20Weight-Bearing%20CT%20for%20Evaluation%20of%20Subtle%20Tibiofemoral%20Syndesmotic%20Injuries%20in%20the%20Elite%20Athlete%22%2C%22creators%22%3A%5B%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Anthony%20C.%22%2C%22lastName%22%3A%22Mok%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Tanner%22%2C%22lastName%22%3A%22Campbell%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Megan%20R.%22%2C%22lastName%22%3A%22Wolf%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Armin%22%2C%22lastName%22%3A%22Tarakemeh%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Bryan%20G.%22%2C%22lastName%22%3A%22Vopat%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Brian%22%2C%22lastName%22%3A%22Everist%22%7D%5D%2C%22abstractNote%22%3A%22Category%3A%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Ankle%3B%20Sports%3B%20Trauma%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Introduction%5C%2FPurpose%3A%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Tibiofibular%20syndesmotic%20injuries%20occur%20in%20association%20with%20fractures%20or%20in%20isolation%2C%20often%20referred%20to%20as%20%26%23039%3Bhigh%20ankle%20sprains%26%23039%3B.%20Dynamic%20instability%20of%20the%20syndesmosis%20in%20the%20high-level%20athlete%20is%20difficult%20to%20diagnose%2C%20requiring%20a%20high%20index%20of%20suspicion%20for%20the%20identification%20and%20surgical%20stabilization%20to%20prevent%20late%20adverse%20sequelae%2C%20such%20as%20recurrent%20ankle%20pain%2C%20anterolateral%20soft%20tissue%20impingement%2C%20local%20synovitis%2C%20and%20chronic%20instability%20of%20distal%20syndesmosis.%20Both%20CT%20and%20MRI%20have%20been%20helpful%20in%20identifying%20syndesmotic%20injuries%2C%20but%20are%20static%2C%20non-weight%20bearing%20imaging%20modalities%20that%20cannot%20demonstrate%20dynamic%20instability%2C%20which%20may%20miss%20the%20unstable%20nature%20of%20some%20injuries.%20We%20present%20a%20novel%20imaging%20technique%20utilizing%20weight-bearing%20CT%20with%20syndesmotic%20stress%20in%20which%20a%20case%20is%20presented%20to%20illustrate%20it%26%23039%3Bs%20utility%20in%20the%20elite%20athlete.%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Methods%3A%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Cone%20beam%20weight%20bearing%20computed%20tomography%20%28WBCT%29%20is%20a%20well-described%20imaging%20modality%20for%20evaluation%20of%20foot%20and%20ankle%20injuries.%20We%20have%20found%20it%20highly%20beneficial%20to%20teach%20our%20CT%20technologists%20about%20syndesmosis%20injuries%20and%20the%20importance%20of%20augmented%20physiologic%20stress%20applied%20across%20the%20ankle%20joint%20%28in%20addition%20to%20the%20standard%20WBCT%29%20to%20better%20demonstrate%20instability%20or%20motion%2C%20particularly%20at%20the%20tibiofibular%20syndesmosis.%20To%20perform%20an%20augmented%20stress%20weight%20bearing%20CT%2C%20the%20patient%20is%20positioned%20in%20the%20WBCT%20scanner%20and%20initial%20images%20are%20acquired%20in%20the%20standing%20weight%20bearing%20position%20with%20feet%20directed%20forward%20and%20weight%20distributed%20equally.%20A%20second%20image%20acquisition%20is%20then%20performed%20%28augmented%20stress%29%2C%20where%20the%20patient%20is%20coached%20to%20firmly%20plant%20both%20feet%20on%20the%20ground%20and%20internally%20rotate%20the%20shin%20and%20knee.%20This%20places%20an%20external%20rotational%20moment%20on%20the%20TFS%20due%20to%20the%20planted%20foot%20and%20ankle.%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Results%3A%5Cn%20%20%20%20%20%20%20%20%20%20%20%20%20%20Our%20case%20is%20of%20a%2021-year-old%20male%20collegiate%20football%20player%2C%20wide%20receiver%2C%20that%20sustained%20an%20eversion%20injury%20to%20his%20left%20ankle%20during%20game%20play.%20He%20subsequently%20was%20unable%20to%20bear%20weight%2C%20with%20pain%20anteriorly%20over%20the%20distal%20tibiofibular%20joint.%20Pain%20was%20reproduced%20with%20dorsiflexion%20and%20eversion%20stress%3B%20however%2C%20he%20had%20a%20negative%20%26%23039%3Bsqueeze%20test%26%23039%3B%20on%20examination.%20Initial%20weight%20bearing%20radiographs%20were%20negative.%20An%20MRI%20was%20obtained%20which%20did%20not%20reveal%20evidence%20of%20syndesmotic%20injury.%20Subsequently%2C%20weight-%20bearing%20CT%20stress%20examination%20of%20the%20syndesmosis%20was%20obtained%20and%20found%20to%20have%208%20mm%20widening%20of%20the%20tibiofibular%20syndesmosis%2C%204%20mm%20increased%20when%20compared%20to%20his%20contralateral%2C%20uninjured%2C%20ankle.%20This%20was%20also%20compared%20with%20a%
